Orlando Health, Inc. seeks a Behavioral Health Care Coordinator for the Lakeland Highlands Hospital opening team, joining Summer 2026.

The role focuses on coordinating care across inpatient, emergency, and outpatient settings to ensure smooth discharge planning and coordinated care for patients with complex needs. Requires a Master’s degree in Social Work or related field, HWC certification, and at least two years of direct clinical behavioral health experience.

Qualifications

• Masters degree in Social Work, Mental Health, Psychology or Marriage and Family Therapy.
• Handle with Care (HWC) Certification required for Behavioral Health Unit.
• Two years of direct clinical experience in the specified population.

Responsibilities

• Coordinate care across inpatient, emergency and ambulatory settings to ensure seamless discharge planning.
• Collaborate with Patient and Family Counselors/Social Workers and UR nurses to support discharge planning.
• Educate patients and families on self-management and navigating the health care system.
• Monitor progress toward discharge plans and adjust as needed.
• Advocate for patients to address safety, financial and other needs.
