Job summary

Orlando Health, Inc. seeks a Care Coordination professional to collaborate with clinical teams in identifying patients who benefit from coordinated care.

The role emphasizes risk assessment, discharge planning, and post-discharge follow-up across inpatient, emergency, and outpatient settings. Required Master’s degree in Social Work or related field and HWC certification for Behavioral Health, plus at least two years of clinical experience or a master’s internship substitution.

Qualifications

• Master’s degree from an accredited school of Social Work, Mental Health, Psychology or Marriage and Family Therapy is required.
• Handle with Care (HWC) Certification required for Behavioral Health Unit.
• Two years of direct clinical experience with emphasis on the served population; Master’s internship may substitute.

Responsibilities

• Takes the lead to ensure continuity of care across inpatient, emergency, and ambulatory settings.
• Collaborates with Patient and Family Counselors/ Social Workers and UR nurses to support discharge planning.
• Monitors progress toward discharge plans and adjusts as needed for high‑risk patients.
• Educates patients and families about self‑management and health‑care system navigation.
• Works with IT resources to facilitate registry reporting and disease outcome measures.
