AdventHealth in Ocala, FL is seeking a qualified Social Worker for discharge planning across inpatient, observation and emergency departments. You will assess patients and families, review records, and coordinate post-acute care to ensure timely and safe transitions.

Requirements include a Master’s in Social Work, 2+ years of social work experience, and a licensed clinical social worker (LCSW) or licensed mental health counselor (LMHC).

Qualifications

• Master's in social work (Required).
• 2+ social work experience (Required).
• LCSW or LMHC license (Required).

Responsabilités

• Assesses patients’ and families’ wholistically for discharge planning needs in inpatient, observation and/or emergency departments.
• Reviews medical records and coordinates post-acute care arrangements.
• Develops discharge plans and ensures timely care coordination across settings.
• Communicates with payors for post-acute care authorizations as needed.
• Educates patients and families about impacts of illness and available resources.
