Orlando Health Melbourne Hospital is seeking a Care Coordination leader to enhance discharge planning, coordinate across inpatient, ED and ambulatory settings, and advocate for patients' needs. The position requires RN licensure with potential MSW/MHC/MFT background and a track record in care coordination.

Responsabilidades

• Takes the lead in ensuring continuity and consistency of care across the continuum to enable integrated delivery and discharge planning.
• Develops working relationships with Licensed Mental Health Counselors/Social Workers and UR nurses to support patient/family needs and timely discharge.
• Identifies patients who could benefit from care coordination and assesses risk factors and needs.
• Serves as mentor to care coordination team through guidance and education.
• Monitors team performance and progress toward discharge plans, adjusting as needed.
• Develops collaborative relationships with site care administrators, medical staff leadership, and community partners.
• Ensures activities meet clinical standards and regulatory requirements; accounts for age, development, and cultural needs.
• Promotes professional conduct and a patient-first mindset.
