Job summary

Central Florida Behavioral Hospital in Orlando seeks a Discharge Planner to ensure smooth transitions from inpatient care to community-based services. You will bridge clinical care and aftercare, coordinating therapy, medications, housing and community supports to reduce readmission risk.

The role requires a Bachelor’s degree in social work or related field, plus background checks and relevant psychiatric experience. Orientation participation is required.

Qualifications

• Bachelor’s degree in social work or related field required.
• Ability to pass a Level 2 Background Investigation.
• Two years’ experience in a psychiatric setting preferred.
• Handle with Care, Verbal Deescalation and BLS/CPR required (may be obtained upon hire).
• Effective oral and written communication skills.

Responsibilities

• Arrange and follow up on post-discharge services such as therapy, medication management, housing, and community support.
• Meet with patients to review the discharge plan and ensure patient understanding.
• Ensure patients leave with prescribed medications and refills and understand how to access them.
• Involve patients and families in planning, providing education, and addressing questions in understandable language.
