Job summary

SSM Health St. Anthony Hospital – Oklahoma City is seeking a PRN RN to assess, develop, implement, and evaluate discharge planning needs in collaboration with the interdisciplinary team.

You will ensure CoP discharge planning compliance, identify barriers to length of stay, and work with post-acute providers to coordinate escalations when needed. The role requires nursing licensure in Illinois, Missouri, Oklahoma, or Wisconsin as applicable, at least three years of acute care experience, and

Qualifications

• Three years’ acute care experience.
• Graduate of accredited school of nursing or education equivalency for licensing.
• RN licensing requirements include state-specific boards.

Responsibilities

• Ensure compliance with CoP discharge planning.
• Identify gaps/barriers related to inpatient/observation length of stay.
• Assess high-risk patients for readmission and collaborate on discharge plans.
• Participate in opportunities for improvement in standard work.
• May handle utilization management, ED case management, admission processes, and liaison with post-acute providers.
• Apply evidence-based practice to nursing care per department scope.
