Job summary

AdventHealth Shawnee Mission is seeking a Care Management Registered Nurse (RN) for PRN scheduling. The RN will coordinate discharge planning, advocate for patients, and collaborate with interdisciplinary teams to ensure safe transitions of care.

The role requires an ADN, RN licensure, and 2+ years in acute care. Preferred BSN and CCM/ACM certifications. Flexible schedule and opportunities for professional growth are offered.

Qualifications

• ADN is required for RN licensure.
• BSN is preferred.
• Two years of acute care or hospital nursing experience required.
• Care Management or Utilization Management experience preferred.

Responsibilities

• Coordinate comprehensive discharge planning for patients during acute care hospitalization.
• Evaluate post-acute care needs and develop individualized transition plans with the team.
• Receive referrals for care coordination and respond promptly.
• Participate in multidisciplinary rounds to review progression and discharge readiness.
• Identify delays in progression and elevate to leadership.
• Educate patients/families on illness impacts and empower participation in decisions.
• Connect patients with community resources and post-acute services.
