You will help coordinate post-discharge services, arrange appointments, and ensure DME and community resources are available. The ideal candidate is organized, communicates clearly with patients and families, and can problem-solve to promote timely, compassionate care.

Qualifications

• High school diploma or equivalent required.
• One year of healthcare experience preferred.
• Strong verbal and written communication skills.
• Computer proficiency.
• Excellent organizational and time-management skills.
• Strong problem-solving and follow-through skills.

Responsibilities

• Coordinate post-discharge services, appointments, and required documentation.
• Collaborate with DME providers to ensure equipment is available at discharge.
• Communicate effectively with patients and families regarding discharge needs.
• Assist with connecting patients to community resources and support services.
• Support team conference documentation and compliance requirements.
• Maintain accurate and timely records.
