Job summary

NYU Langone Hospital - Brooklyn is seeking a Case Management Assistant to provide administrative support to the Department of Case Management. The role works under the direction of the Assistant Directors to help develop and implement post-acute discharge plans for patients.

The ideal candidate has a Bachelor's degree and at least 1 year in a consumer service setting, with strong communication, organization, and teamwork skills.

Qualifications

• Bachelor's degree required in any field.
• Minimum 1 year of service in a consumer service role.

Responsibilities

• Performs other related duties as assigned or requested.
• Assists in documenting post-acute discharge plans for patients.
• Collaborates with the interdisciplinary team to implement discharge plans.
• Supports case managers, social workers, and team members in discharge planning matters.
• Screens inquiries and routes to appropriate referrals.
• Inputs data from third party payors into Allscripts.
• References community resources to connect patients with services for continuity of care.
• Obtains approvals for hospital-provided prescriptions at discharge.
• Assembles resources on benefits and entitlements available to patients.
• Works with financial counselors on Medicaid, Medicare, and other resources.
• Maintains professional documentation per policy.
• Assists in collecting and reporting timely statistics.
• Provides information on services and programs offered by NYU Langone HospitalBrooklyn.
• Completes eligibility documents and assessment forms as needed.
