We have an exciting opportunity to join our team as a Nurse, Care Management - Full-Time, Days. The role coordinates services for post-acute care needs and ensures smooth transitions to appropriate settings. Strong collaboration with care teams and payer processes is essential.

Qualificações

• Bachelor's degree in Nursing and NYS Registered Nurse License.
• Current BLS certification (AHA).
• 3-5 years clinical acute care experience.
• Excellent communication, interpersonal and organizational skills.

Responsabilidades

• Performs initial utilization review of patients for appropriateness of setting using industry standard clinical criteria (e.g., InterQual, MCG).
• Provides patient clinical reviews to payer to obtain certification of patient days and ensure reimbursement.
• Screens inpatients for post-acute care needs and develops discharge plans with patient/designee.
• Completes documentation using Allscripts and PRI accurately and timely.
• Identifies and reports risk and quality issues to the appropriate department.
