In this role, the successful candidate working under the RN Care Manager and LCSW provides assistance in setting up post-hospital care and services, ensuring documentation and information required for discharge are complete, and aiding in coordinating reviews with payors.

Qualifications

• High School Diploma or GED required or equivalent.
• Knowledge of medical terminology and health information.
• Proficient in Word processing, spreadsheets, and email.
• Strong interpersonal, organizational, and communication skills.

Responsibilities

• Participate in discharge planning to ensure timely patient discharge.
• Assist with intake, tracking, and communication with payers.
• Maintain case files and verify health insurance information.
• Coordinate transportation, DME, and community services for post-acute care.
