Job summary

NYU Langone Hospital—Suffolk is seeking a Care Management Assistant to join our full-time day shift team. Working under the RN Care Manager and LCSW, you will help set up post-hospital care and ensure discharge planning documentation is complete.

You will support case managers by maintaining files, coordinating post-acute referrals, and communicating with payors and providers to enable timely discharges. Strong medical terminology knowledge and solid organizational skills are essential.

Qualifications

• High School Diploma or GED required.
• Knowledge of medical terminology.
• Proficiency with word processing, spreadsheets and email.
• Excellent interpersonal skills to communicate with internal and external customers.
• Strong organizational and analytical abilities to manage multiple tasks.
• Ability to work in a team.
• Self-directed and able to work independently.
• Excellent verbal and written communication skills.

Responsibilities

• Assist in discharge planning activities to ensure timely patient discharge and linkage with post-acute providers.
• Support the discharge planning process by handling intake, tracking and communication with payors.
• Maintain case files, update referral information, and verify health insurance or benefits details.
• Coordinate transportation, DME, and community services as directed by care managers and social workers.
