Job summary

Endeavors is seeking a Disaster Case Manager to support individuals and families affected by disasters. You will conduct outreach, complete eligibility assessments, perform home and community visits, and collaborate to develop recovery plans that address unmet needs and map to available benefits and services.

You will monitor progress, document case history accurately, coordinate with health professionals and community partners, and provide regular updates to supervisors.

Qualifications

• Bachelor's degree in behavioral sciences, human services, or social services preferred.
• High School diploma or GED with 3 years of relevant disaster case management experience.
• Bilingual English/Spanish skills preferred.

Responsibilities

• Conduct outreach to disaster-affected clients.
• Complete eligibility assessments and intake processes.
• Perform home and community visits to provide ongoing support.
• Develop recovery plans addressing unmet disaster-caused needs.
• Monitor progress and evaluate service effectiveness.
• Identify and connect clients to benefits, resources, and services.
• Maintain accurate client records and documentation.
• Collaborate with stakeholders to address client needs.
