Job summary

Endeavors Inc. is seeking a Disaster Case Manager to support individuals and families recovering from disasters. You will conduct outreach, assess needs, and develop recovery plans while guiding clients to available resources and services.

You will document case histories, travel to client sites, and work with a multidisciplinary team to monitor progress and outcomes, ensuring compassionate, timely service and compliance with program requirements.

Qualifications

• Bilingual communication skills (English/Spanish) preferred.
• Bachelor's degree in behavioral sciences, human services, or social services preferred or equivalent experience.

Responsibilities

• Conduct outreach to disaster-affected clients.
• Complete eligibility assessments and the intake process.
• Conduct home and community visits to provide ongoing support.
• Collaborate with clients to develop recovery plans addressing unmet disaster-caused needs.
• Monitor progress and assess the effectiveness of services through follow-up visits.
• Assist clients in identifying and securing available benefits, community resources, and social services.
• Maintain accurate and detailed records of client background, case history, and progress toward recovery goals.
• Utilize online software to document and track case information.
• Prepare reports as requested by the Case Management Supervisor or Program Manager.
