Qualifications

• High School Diploma or GED required.
• Minimum of one year working with individuals with disabilities.
• Ability to drive a 15-passenger van and be insurable.

Responsibilities

• Provide direct support to individuals per ISPs and HCBS standards.
• Monitor daily progress toward ISP goals and document outcomes.
• Maintain daily schedules of training, activities, and skill-building.
• Assist with transitions, personal care, and health needs as needed.
• Support medication management and safety protocols.
• Develop and implement independence-promoting activities.
