Job summary

United Cerebral Palsy of Eastern Connecticut (UCP of Eastern CT) is seeking a Case Manager to coordinate in-home support services for individuals with IDD. You will advocate for clients, manage a caseload, ensure compliance, and assist with budgeting, benefit applications, and health oversight.

Under the Director of Residential Services, you will collaborate with families, DSPs, and medical staff; document activities, conduct team meetings, and support safety, independence, and community access

Qualifications

• Prior experience providing direct care to individuals with IDD, required.
• Prior experience in case management, advocacy, or support coordination role, preferred.
• Knowledge of Medicaid waivers, ISPs, and person-centered planning, preferred.
• Experience collaborating with interdisciplinary teams, including medical professionals, therapists, and direct support staff.

Responsibilities

• Coordinate and oversee in-home supports for individuals with IDD.
• Advocate for individuals' rights, access to services, and community inclusion.
• Monitor health, medications, budgeting, and benefit applications; ensure program compliance.
