Old Pueblo Community Services is seeking a Case Manager I in Tucson, AZ to engage, assess, and coordinate care for individuals experiencing homelessness and related conditions. Duties include intake, recovery planning, risk assessment, and life skills support.

The role requires a high school diploma or GED, CPR/First Aid certification, basic computer skills, and a willingness to learn evidence-based practices. Strong teamwork and reliable transportation are essential.

Qualifications

• High School Diploma or GED required.
• CPR/First Aid certification or ability to obtain.
• Basic computer skills including Word, Excel, and email.
• Willingness to learn evidence-based practices (CPI, Motivational Interviewing, Housing First, Harm Reduction).
• Self-starter with teamwork, dependable and able to manage priorities.

Responsibilities

• Complete intake with new clients.
• Deliver services per recovery plan to meet goals.
• Update recovery plan with Case Manager II as services progress.
• Complete C-SSRS and coordinate risk plans with supervisor.
• Connect clients to mainstream services and other agencies.
• Provide case management and life skills sessions.
• Co-facilitate life skills groups with Case Manager II+.
• Facilitate ART meetings as needed with supervisor guidance.
• Document services in the EHR.
• Maintain client files with HIPAA compliance.
• Monitor client compliance with program guidelines.
• Collaborate with OPCS team and Home Health Care partners.
