Job summary

SCAN Health Insurance is seeking a Director of Value-Based Care Partner Success to lead strategic partnerships with provider organizations and optimize performance of value-based arrangements. You will collaborate with General Managers, Provider Relations, Network Management, and Finance to improve clinical, operational, and financial outcomes.

The role oversees onboarding, governance structures, and program deployment, while driving innovation and adoption of care management and population

Qualifications

• 8+ years of progressive healthcare experience in value-based care, provider partnerships, population health, or healthcare operations.
• 5 years of leadership experience managing programs, teams, or strategic initiatives.
• Bachelor’s Degree required in Healthcare Administration, Business, Public.
• Graduate or Advanced Degree in MBA, MHA, MPH, or related field is preferred.

Responsibilities

• Value-Based Care Partnership Leadership: serve as strategic partner to provider organizations in value-based programs.
• Joint Operating Committee (JOC) Leadership and Governance: lead design and facilitation of JOCs and governance.
• New Partner Implementation and Readiness: design and execute playbooks for onboarding new providers.
• Population Health Strategy and Innovation: identify needs and translate insights into actionable initiatives.
• Value-Based Contract Assessment and Development: provide strategic recommendations for partnership readiness and contract design.
• Cross-Functional Leadership: coordinate clinical, operational, analytics, finance, and provider-facing teams.
