Job summary

SCAN Group is seeking a Director, Value-Based Care Partner Success in California to oversee partnerships and assure performance across networks. This leader will work with GM, Network Management, Provider Relations, Clinical Operations, and Finance to drive joint accountability for outcomes.

The role requires 8+ years in healthcare value-based care and exceptional executive communication, with a focus on onboarding, governance, and population health initiatives.

Qualifications

• 8+ years of progressive healthcare experience in value-based care, provider partnerships, managed care, population health, or healthcare operations.
• 5 years of leadership experience managing programs, teams, or strategic initiatives.
• Experience working with risk-bearing provider organizations, medical groups, IPAs, ACOs, or health systems.
• Strong understanding of value-based reimbursement models, risk arrangements, provider economics, and population health management.

Responsibilities

• Lead strategic oversight of value-based care provider partnerships.
• Coordinate onboarding, governance, and performance improvements for value-based arrangements.
• Build executive-level relationships with physician leaders and health systems.
• Lead design and governance of Joint Operating Committees for VBC partnerships.
• Develop and implement a New Partner Playbook for onboarding providers.
• Identify opportunities to advance innovation and value-based care capabilities.
