Job summary

SCAN is seeking a Director of Value-Based Care Partner Success in California to oversee strategic partnerships, governance, and performance across provider organizations. This leadership role partners with General Managers, Network Management, Provider Relations, and Finance to align clinical, operational, and financial outcomes.

You will drive onboarding of new partners, design and maintain a comprehensive partnership playbook, and lead Joint Operating Committees to improve population health,

Qualifications

• 8+ years of progressive healthcare experience in value-based care, provider partnerships, managed care, population health, or healthcare operations.
• 5 years of leadership experience managing programs, teams, or strategic initiatives.
• Bachelor’s Degree in Healthcare Administration, Business, Public health (or related field).
• Experience with risk-bearing provider organizations, medical groups, IPAs, ACOs, or health systems.
• Strong understanding of value-based reimbursement models, risk arrangements, provider economics, and population health management.
• Demonstrated success leading cross-functional initiatives and provider-facing programs.
• Strong analytical, financial, and executive communication skills.

Responsibilities

• Lead value-based care partnership leadership with provider organizations.
• Coordinate governance and performance reporting for JOCs.
• Oversee new partner onboarding and readiness playbooks.
• Drive population health strategies and innovation with Clinical Center of Excellence.
• Advise on contract design and risk-sharing models.
• Lead cross-functional teams across clinical, operational, analytics, finance, and provider-facing units.
