Job summary

Promise Healthcare is seeking a Director of Population Health to lead design, execution, and evaluation of quality improvement and population health strategies across PHC. This role drives advanced analytics, data-informed decisions, and cross-department initiatives to improve access, outcomes, and patient satisfaction.

The ideal candidate will hold a Master’s in a health field, 5+ years in healthcare with leadership experience, and strong skills in data visualization tools such as Power BI and

Qualifications

• Master's degree in public health, health administration, nursing, social work, or related field; equivalent experience considered.
• 5+ years of healthcare/public health experience with at least 2 years in a leadership or managerial role.
• Knowledge of population health, health equity, value-based care models, and quality improvement strategies.
• Experience with FQHCs or similar safety-net settings preferred.
• Advanced data literacy with experience in population health analytics and translating data into actionable insights.

Responsibilities

• Lead vision and strategy for population health program to support clinical quality outcomes, equity, and financial sustainability.
• Recruit, train, and supervise population health interns and coordinate with quality staff.
• Partner with clinical leaders to advance integrated population health strategies and accountability.
• Develop relationships with MCOs and payors to optimize incentive programs and value-based agreements.
• Lead analysis of clinical quality, utilization, and social determinants data to identify disparities and forecast trends.
• Oversee dashboards and reports communicating health trends to stakeholders.
