This role involves developing strategies to enhance care quality, overseeing compliance metrics like HEDIS, managing care coordination, and leading interdisciplinary teams toward better patient outcomes.

The salary range for this position is $145,000-$175,000.

Qualifications

• Minimum of 7–10 years of progressive leadership experience in population health or care management.
• Demonstrated success in implementing HEDIS or CMS Star Ratings.
• Proven ability to lead interdisciplinary teams and manage complex initiatives.
