Job summary

Penn Medicine is seeking a Director of Population Health Quality Outcomes to design, implement, and lead enterprise-wide quality improvement initiatives across the primary care and specialty networks. The role focuses on improving patient outcomes through proactive outreach, closing care gaps, and maximizing payer incentives in value-based arrangements.

Reporting to the COO, Population Health, the director will drive standardization, dashboard-driven performance, and strategic collaboration

Qualifications

• Bachelor's degree required in Nursing, Pharmacy, Physical Therapy or Healthcare administration.
• 10+ years of healthcare experience with 8+ years in leadership/management.
• Experience in population health or value-based care operations; familiarity with MSSP/ACO is a plus.

Responsibilities

• Lead enterprise-wide quality improvement initiatives to support population health goals.
• Design and implement programs to close preventive care gaps and standardize care delivery.
• Leverage dashboards and analytics to maximize payer incentives and outcomes.
• Partner with leaders across hospitals and primary care to improve access and reduce avoidable utilization.
• Ensure compliance with CMS, Joint Commission, NCQA regulations and internal standards.
