Job summary

Apicha Community Health Center in New York, NY seeks a Population Health Manager to drive value-based care initiatives and coordinate population health projects. You will analyze data, manage quality programs, and collaborate with multidisciplinary teams to meet clinical metrics and improve health outcomes.

Qualifications

Bachelor’s degree in Nursing, Health Care Administration, or related field required; advanced degree strongly preferred.
Experience managing projects in health care quality improvement, performance improvement, and/or practice transformation.
Intermediate to advanced analytic skills and knowledge of research methods, data collection, and analytical methodologies.
Familiarity with medical terminology, ICD-10, and CPT coding; understanding of UDS, HEDIS, and NCQA PCMH accreditation standards.
Excellent verbal and written communication skills; detail oriented; proofreading and editing skills.
Strong working knowledge of Microsoft Office applications (Word, Excel, PowerPoint).
Ability to work independently and in a team with multidisciplinary staff.

Responsibilities

Assist in project scopes and statements of work under the guidance of the Director of Population Health Management.
Monitor project progress by tracking milestones, resolving problems, and coordinating status update meetings with stakeholders.
Develop workplans and protocols to guide team activities and ensure delivery of KPIs.
Coordinate data collection and reporting of member enrollment and quality indicators with value-based initiatives.
Analyze healthcare data to identify best practices, care delivery trends, and clinical variation.
Utilize Excel and BI tools (Tableau, Power BI, SAS) to prepare presentations and reports.
Validate data accuracy and support QA efforts including NCQA PCMH, HEDIS, UDS, HIV constructs.
Lead QA/QI initiatives and assist in RCA, PDSA, DMAIC methodologies.
Supervise Quality Improvement Coordinator and collaborate with clinical staff.
