Responsibilities

• Lead PCC service delivery to ensure safe, timely patient care and compliance with standards.
• Oversee prescription refill workflows, diagnostic test result communication, and nurse triage.
• Collaborate with physicians, clinic operations, IT, Pharmacy, Quality and other departments.
• Develop and execute strategic and tactical operational plans; monitor metrics and adjust as needed.
• Ensure alignment with organizational goals for patient safety, quality, efficiency, and experience.
