Job summary

Complete Care in Brick Township, NJ is seeking a Director of Social Services (LSW/CSW) to lead the Social Services Department, address residents' psychosocial needs, and support families throughout the healthcare journey.

You will participate in interdisciplinary care planning, ensure compliant documentation, advocate for residents, coordinate admissions and discharge planning, and guide social services staff. Equal Opportunity Employer.

Qualifications

• Current LSW or CSW required.
• Long-term care experience preferred, but not required.
• Prior supervisory or management experience preferred.
• Current AHA BLS CPR Certification recommended.
• Strong communication, organizational, and documentation skills.
• Computer proficiency required.
• Ability to work independently and as part of an interdisciplinary team.

Responsibilities

• Lead and oversee the Social Services Department at Complete Care.
• Develop and implement social service programs, policies, and goals.
• Advocate for residents' psychosocial, emotional, and behavioral health needs.
• Participate in interdisciplinary care plan meetings and collaborate with the clinical team.
• Maintain accurate, timely, and compliant documentation.
• Educate residents and families regarding Advance Directives and available resources.
• Coordinate admissions, transitions, and discharge planning to ensure positive outcomes.
• Provide guidance and support to social services staff.
• Assist residents and families with community resources and support services.
• Perform additional duties as assigned by the Administrator.
