Job summary

The Blossoms is seeking a Director of Social Services to plan and direct the social service department in accordance with federal, state, and local regulations. You will oversee administrative, committee, personnel, and safety functions, coordinate care plans, discharge planning, and community partnerships while maintaining quality service and a patient‑centered approach.

Responsibilities include policy implementation, staff supervision, budgeting, and ensuring compliance with accreditation

Qualifications

• Must possess a Bachelors Degree in Social Work and be a member in good standing of the NASW/ACSW.
• Must have supervisory experience in a hospital, skilled nursing care facility, or related medical facility.
• Must read, write, speak, and understand English; excellent communication and interpersonal skills.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct the social service programs of the facility.
• Administer and coordinate department policies and procedures.
• Oversee discharge planning and development of social care plans for patients.
• Coordinate with administration, medical, nursing, and other departments to ensure quality social services.
• Maintain a quality assurance program for the social services department.
