Job summary

The Blossoms in Van Buren, AR is seeking a Social Services Assistant to support residents’ social and emotional well-being within our long-term care setting. You will help develop care plans, coordinate admissions and transfers, and participate in interdisciplinary team meetings.

The ideal candidate has a Bachelor's degree in a social field and at least one year of social work experience, preferably in long-term care.

Qualifications

Responsibilities

• Maintaining and addressing residents' social and psychosocial needs.
• Identifying individual social and emotional needs.

• Attending interdisciplinary care plan meetings for social needs.
• Collaborating with finance to ensure proper documentation.
• Coordinating discharge planning and post-care arrangements.

Key Responsibilities

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Skills, Knowledge and Expertise

• Bachelors degree Social field is required
• At least one year experience in Social Work field required
• Experience in long term care and/or sub-acute setting preferred
