The Blossoms is seeking a qualified Social Worker (BSW/MSW) to support residents in long-term care settings in Arkansas. You will assess social and emotional needs, develop individualized care plans, and participate in interdisciplinary teams to ensure smooth admissions, transfers, and re-entries into the community.

Responsibilities include discharge planning, coordinating DME or home care, and collaborating with the finance team to ensure proper documentation.

Qualifications

• BSW or MSW required, or Bachelors degree in related field.
• At least one year experience in Social Work field required.
• Experience in long term care and/or sub-acute setting preferred.

Responsabilités

• Maintaining and/or improving resident’s ability to control daily needs and mental/psychosocial needs.
• Identifying individual social and emotional needs.
• Developing and maintaining individualized care plans.
• Participating in admission planning, community re-entry or transfer to another facility.
• Participating in interdisciplinary care plan meetings to address social and emotional needs.
• Work with the finance team to ensure resident education and proper documents.
• Assist with discharge planning and arranging DME/home care for community return.
