Responsibilities

• Conduct psychosocial assessments.
• Develop care plans and update interdisciplinary team on resident progress and goals.
• Provide emotional support to help residents cope with grief, loss, depression, anxiety, and transition to a nursing home.
• Advocate for resident rights, ensuring voices are heard and needs are met.
• Mediation of complaints and reporting suspected abuse or neglect.
• Coordinate discharge plans, connecting residents with community resources such as home health care or hospice.
• Assist with connecting residents and families to legal, financial, and insurance case management.
• Guide residents and families through advanced directives and end-of-life support.

Qualifications

• Licensed to work within the State of Connecticut as a LSW.
• Ability to pass background check, drug screen, professional references check, and physical evaluation.
• High level understanding of leadership, customer service, and clinical competency.
• Leadership experience in a long-term care setting.
