Job summary

Cambridge Healthcare Services, LLC is seeking a Director of Social Services to lead planning, development, implementation, and evaluation of resident social services programs in a California facility. Qualifications

• Bachelor’s Degree in Social Work or a bachelor’s degree in a human services field including but not limited to sociology, gerontology, special education, rehabilitation counseling, and psychology; and one year supervised social work experience in a health care setting working directly with individuals required for facility with more than 120 beds.
• Bachelor’s Degree in Social Work or Social Service Designee Certification with at least 1 year experience for facilities under 120 beds.
• Minimum of 1 year of experience in social services skilled nursing setting
• Minimum of 1 year of supervisory experience in skilled nursing
• Excellent English language usage in writing, speaking and presenting
• Proficiency with Microsoft Word, Excel, and PowerPoint
• Knowledge of Point Click Care a plus

Responsibilities

• Complete all required social services charting, including Minimum Data Set (MDS), psychosocial evaluations, social histories, assessments, quarterlies, updates as needed and Patient Care Plan (PCP) entries to meet federal/state licensing requirements, facility policies and procedures and to accurately reflect Resident needs
• Discharging, appointment setting and arranging transportation for residents
• Managing, scheduling, mentoring and hiring staff
• Adhering to the facilities operational budget
• Participate as a member of the Interdisciplinary Team (IDT)
• Participating in facility surveys
• Compiling and maintaining the social service directory listing
• Assisting the QA Committee in appropriate plans of correction if warranted
• Keeping updated with news laws and regulations as they pertain to resident care
• Following Infection and Control policies and Safety and Sanitation procedures
• Reporting any occupational exposures to blood, body fluids, or other hazardous materials immediately
• Protecting each residents file and data with confidentially per HIPAA standards
• Abiding with all facility policies and procedures including not disclosing user ID codes and passwords
