Job summary

Regalcare in Massachusetts is seeking a Director of Social Services to lead our social services program in a skilled nursing facility. You will oversee assessments, care planning, discharge planning, and coordination with families and the care team to enhance residents’ quality of life.

The role requires a strong background in social work within long-term care, MSW preferred, and at least two years of relevant experience, with solid knowledge of MDS 3.0 and regulatory compliance.

Qualifications

• Bachelor's Degree in Social Work or related field required; MSW preferred.
• Minimum 2 years of experience in long-term care or healthcare.
• Familiarity with MDS 3.0 and care planning process.
• Strong communication, organizational, and interpersonal skills.

Responsibilities

• Coordinate all aspects of social services in the facility.
• Serve as liaison between residents, families, and the care team.
• Conduct psychosocial assessments and create care plans.
• Assist residents and families with discharge planning and referrals.
• Ensure compliance with federal, state, and local regulations.
• Maintain accurate documentation and resident records.
• Provide support and guidance to social work team members.
