Jewish Community Services (JCS) is seeking an experienced and compassionate Full-Time Manager, Elder Care Management to lead our Elder Care Management Program. This hybrid role focuses on aging services, care management, program operations, and staff supervision to deliver high-quality, person-centered care for older adults and their families.

The role includes program leadership, community outreach, compliance, and fiscal stewardship, with a small caseload and collaboration with agency

Qualifikationen

• Master's or doctoral degree in Social Work, Gerontology, Human Services, or a related field required.
• Three to five years of relevant professional experience in aging services, care management, or a related field.
• Minimum of three years of supervisory or management experience.
• Strong knowledge of elder care management processes, assessments, care planning, and service coordination.

• Lead and oversee the Elder Care Management Program and senior services.
• Recruit, supervise, and evaluate Elder Care Managers and program staff.
• Maintain a small caseload and ensure high-quality, person-centered care.
• Develop partnerships and enhance community outreach and collaboration.
• Monitor compliance with policies and regulatory requirements.
• Oversee program budgets, grants, and fiscal activities.
• Drive quality improvement and measure program outcomes.
• Collaborate with agency leadership on strategic planning and growth.
