RRHS seeks a Director of Case Management in Rutherfordton, NC to oversee the Case Management unit, including discharge planning, patient advocacy, and clinical social work. The role focuses on evidence-based care management, resource optimization, and regulatory adherence with a goal of improving hospital performance metrics.

The Director will lead across departments, coordinate care transitions from hospital to community, and participate in strategic planning and performance improvement

Formación

• RN license to practice in NC (NC licensure or multi-state (compact) license).
• Associate's Degree in Nursing is required.
• CCM Certification within 1st year of employment or transfer to the role.

Responsabilidades

• Oversee Case Management unit and ensure clinical resource management and discharge planning.
• Lead care coordination across departments and with providers and community programs.
• Ensure compliance with regulatory and accreditation standards (Joint Commission, CMS, HCFA).
• Drive performance improvement and LOS reduction initiatives.
• Collaborate with physicians, nurses, and social services to optimize patient care transitions.
