PAM Health is seeking an experienced Director of Case Management (RN) for a full-time, day-shift role in a hospital setting. The position oversees the hospital-wide case management program, supervising staff, discharge planning, and care coordination to optimize patient outcomes and costs.

Required RN licensure in the applicable state, with BSN/MSN preferred. Three to five years in case management and prior IRF/LTACH or management experience are valued; various certifications may be considered

Qualificações

• RN licensure in state of hospital required.
• Social workers may be accepted in lieu of RN licensure.
• MSW, LSW, PT, OT, SLP and RT licensure may be accepted per state regulations.
• Current BLS certification required.
• BSN/MSN preferred.
• Certification in an approved Case Management Program preferred.

Responsabilidades

• Coordinate hospital-wide case management program and oversee admissions review for appropriate utilization.
• Assist case managers with timely care coordination and discharge planning.
• Supervise case managers and daily department operations to optimize financial and clinical outcomes.
• Ensure screening of admissions to determine level of care.
• Perform fiscal planning and regulatory compliance tasks.
• Analyze systems and variances to identify improvement opportunities and promote quality of care.
