RWJBarnabas Health seeks an Assistant Director of Case Management RN to lead the Case Management department at Williamson Street in Elizabeth, NJ. The role oversees daily operations, patient flow, and utilization management, while guiding discharge planning and interdisciplinary coordination to improve outcomes.

The ideal candidate has 4+ years clinical hospital/home health experience, 2+ years supervisory leadership, and strong data-driven decision skills.

Qualifications

• Bachelor's degree in Nursing or MSW required.
• Current, valid RN or LCSW license in New Jersey.
• Master's degree preferred for RN (MS/NHA) while CSW path acceptable with LCSW.
• ACM/CCM certification preferred and must be obtained within one year of hire.
• Minimum four years of clinical hospital/home health experience with two years in case management and two years of supervisory leadership.

Responsibilities

• Provide leadership and operational oversight for Case Management services.
• Manage staffing, assignments, workflows, and resource allocation.
• Design staffing models to improve throughput and efficiency.
• Identify barriers to patient progression and reduce delays.
• Collaborate with physicians and care teams on utilization review and discharge planning.
• Review KPIs and drive performance improvements in length of stay and readmission rates.
• Coach and mentor Case Management staff and lead recruitment and retention efforts.
• Develop and enforce policies to ensure regulatory compliance and best practices.
• Coordinate with interdisciplinary partners to support transitions of care.
