Mission Hospital McDowell, part of HCA Healthcare, is seeking a Director of Case Management to lead the department, align patient care with discharge planning, and coordinate across hospital teams. The director will oversee operations, staffing, and performance improvement initiatives.

The ideal candidate holds a Bachelor's in nursing or health administration, a Master's preferred, RN or LCSW preferred, and 3+ years in case management with leadership experience.

Qualifications

• Bachelor's degree required.
• Master's degree in Nursing, Health Administration, or Business Administration preferred.
• RN or LCSW credentials preferred; travel not required.
• 3+ years in hospital case management; 2+ years in leadership preferred.
• Based at Mission Hospital; travel 2–3 times weekly.

Responsibilities

• Directs and evaluates departmental operations, including CM model, staffing and technology use to hit performance goals.
• Allocates resources to staff the department to meet productivity and quality targets.
• Oversees CM core functions and practice within the department.
• Ensures care coordination and discharge planning processes are timely and well documented.
• Collaborates with CFO and Division CM Leader to meet goals and expectations.
• Assesses performance and implements sustainability improvements using data.
• Ensures compliance with policies and SOPs.
• Builds relationships with CMO, CNO, ancillary leaders, Ethics, Compliance and Legal.
• Performs other duties as assigned; adheres to Code of Conduct and Values.
• Maintains executive presence and drives change across stakeholders.
