Job summary

Ascension, based in Milwaukee, is seeking a Case Management leader to formulate and implement operational strategies within the program at Ascension St. Mary’s. This role directs services, including care plan development and complaint management, to meet performance and quality targets.

Qualified candidates will have 5+ years of experience, leadership experience, and relevant licensure or credentials. The position offers a comprehensive benefits package and an annual bonus incentive.

Qualifications

• Minimum requirements include licensure or credentials: RN (Wisconsin Board of Nursing) or social work credentials; multi-state license acceptable.
• Preferred credentials include ACMA or CCMC Case Manager certification.
• Education: high school diploma with experience, associate/technical degree, or master’s preferred.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
  • One or more of the following:
    • Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
    • Social Worker credentialed from the Washington Mental Health Counselors, Marriage - Family Therap. and Social Workers Advisory Committee obtained prior to hire date or job transfer date.
    • Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
• Preferred Credential(s):
  • Case Manager credentialed from the American Case Management Association (ACMA).
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Education

• High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required. Master's degree preferred.

Work Experience

• 5 years of experience required. 10 years of experience preferred. 2 years of leadership or management experience required. 5 years of leadership or management experience preferred.
