Ascension seeks a nurse to coordinate the overall interdisciplinary plan of care for patients from admission to discharge. The role emphasizes collaboration with care teams, evaluating medical necessity, and advocating for needed resources to ensure smooth transitions.

Required is a nursing diploma with Wisconsin licensure (or multi-state license), with BLS preferred and CCMC credential expected. This role supports the organization’s mission and patient-centered care delivery.

Qualifications

• RN diploma in nursing required.
• Wisconsin Board of Nursing licensure (or current home state license for multi-state) required.
• BLS Provider credential preferred (American Heart Association or American Red Cross).
• CCMC Case Manager credentialed (preferred).

Responsibilities

• Monitors patient medical necessity and level of care through assessments and records.
• Advocates for resources and removal of barriers.
• Maintains dialogue with supervisor and care transition team to implement and reevaluate the health plan.
• Acts as a resource for medical record documentation and care options for post-discharge care.

• Creates plan for care across the continuum, integrating patient/family preferences and values.
• Monitors patient medical necessity and level of care through assessments, ongoing evaluations and/or patient records.
• Advocates for resources and removal of barriers.
• Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
• Acts as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.

What minimum requirements you’ll need

• Required Credential(s): Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
• Preferred Credential(s): BLS Provider. American Heart Association or American Red Cross accepted.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
• Education: Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
