Ascension St. Francis Hospital in Milwaukee, WI is seeking a Behavioral Health professional for the Day Shift, full-time.

You will design care journeys, provide crisis and psychosocial support, advocate for patients, and collaborate with care transitions teams to optimize health plans and documentation. Qualified candidates will have Wisconsin licensure in counseling or social work, or CCMC case management preferred; education includes a high school equivalency with experience or an associate

• CCMC Case Manager credential preferred.

Responsibilities

• Design personalized care journeys across the continuum of care, incorporating patient and family preferences.
• Provide crisis and psychosocial support with tailored treatment recommendations.
• Advocate for patients by addressing barriers and securing resources for care transitions.
• Drive cross-functional alignment with supervisors and care transition teams for proactive health plans.
• Guide post-discharge documentation and determine appropriate levels of care and treatment options.

How you’ll make an impact in this role

• Design personalized care journeys across the full continuum of care, integrating patient and family preferences to ensure compassionate, values-based outcomes.
• Serve as a trusted expert in crisis and psychosocial support, evaluating complex needs from substance abuse to legal or adjustment challenges and creating tailored treatment recommendations.
• Fiercely advocate for patients by identifying systemic barriers and securing critical healthcare and community resources to enable smooth care transitions.
• Drive cross-functional team alignment through continuous dialogue with supervisors and care transition teams, ensuring health plans are proactively evaluated and adapted.
• Guide post-discharge success and documentation excellence by advising care teams on accurate medical records, complex levels of care, and diagnostic-driven treatment options.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• One or more of the following required:
  • Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education

High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
