Job summary

Ascension Health in Milwaukee seeks a Registered Nurse to coordinate the interdisciplinary plan of care from admission to discharge, tailoring care to patient and family values.

The role monitors medical necessity, advocates for resources, and collaborates with the care transition team to ensure correct documentation and appropriate post-discharge options.

Qualifications include Wisconsin RN licensure, BLS and preferred CCMC certification; a nursing diploma is required.

Qualifications

• Registered nurse licensure from Wisconsin Board of Nursing.
• Diploma from an accredited school/college of nursing.
• BLS Provider credential preferred.
• CCMC Case Manager credential preferred.

What minimum requirements you’ll need

Licensure / Certification / Registration

• Required Credential(s):
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
• Preferred Credential(s):
• BLS Provider. American Heart Association or American Red Cross accepted.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Education

• Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
