University Health is seeking a Director of Care Coordination for the Palo Alto Hospital campus. This leadership role drives inpatient care coordination programs and guides transitions of care with community resources to support patients and families.

The candidate will lead a culture of excellence, focus on patient experience, and identify opportunities for improvement across the system, collaborating with senior leaders to implement changes.

Qualifications

• Bachelor’s degree in nursing or social work required; Master’s degree preferred.
• Minimum 3 years’ experience in case management/care coordination.
• Previous nursing or social work inpatient experience required.

Responsabilités

• Develops and oversees programs to support inpatient care coordination and guides transitions of care with community resources.
• Provides strong leadership to foster a culture of excellence in patient experience and quality.
• Identifies opportunities for improvement based on best practice and communicates findings to leadership.
