Job summary

University Health Retama Hospital is seeking a Director of Care Coordination to lead inpatient care coordination programs and guide transitions of care for patients and their families. This senior role will drive a culture of excellence in patient experience and quality across the hospital.

You will oversee program development, collaborate with multidisciplinary teams, and identify improvement opportunities to enhance outcomes and care transitions for both adults and children in a growing

Qualifications

• Bachelor’s degree in nursing is required.
• Master’s degree in nursing is strongly preferred.
• A minimum of three years’ experience in case management/care coordination is required.
• A minimum of three years recent management experience is required.
• Previous nursing inpatient experience is required.

Responsibilities

• Develops and oversees programs to support in-patient care coordination and guides transitional services post hospital discharge with community-based resources to support the social and environmental needs of the patients and their families.
• Provides strong leadership to foster and promote a comprehensive culture of excellence regarding patient experience and quality system wide.
• Identifies opportunities for improvement based on best practice and proactively reaches out to appropriate leadership to communicate findings and initiate needed changes.
