Job summary

University Health is seeking a departmental leader to oversee daily operations and guide staff in care coordination and transitions of care. You will develop programs to support inpatient and post-discharge services, working with community resources to address social and environmental needs of patients and families.

The role emphasizes improving patient experience, quality, and alignment with the health system’s strategic plan, requiring strong collaboration with leadership and frontline teams.

Qualifications

• Bachelor’s degree in nursing, social work, or health science related field required.
• Master’s degree in nursing, social work, business, communications, or health science related field strongly preferred.
• A minimum of three years’ experience in case management/care coordination is required.
• A minimum of three years recent management experience is required.
• Previous nursing or social work clinical inpatient experience is required.
• Additional experience in outcomes, case management, care coordination, transitions of care, utilization management, and managed care is preferred.

Responsibilities

• Provides leadership to assigned staff to meet department operational needs.
• Oversees daily department operations and coordinates activities.
• Develops programs to support in-patient care coordination and transitions of care with community resources.
• Communicates findings and initiates needed changes with leadership.
• Fosters a culture of excellence regarding patient experience and quality system wide.
• Supports the University Health strategic plan.
