Job summary

University Health is seeking a Director of Care Coordination for the Retama Hospital campus. This leadership role will drive care coordination programs, oversee transitions of care, and partner with hospital leadership to enhance patient outcomes and experience.

The role requires a nursing or social work background with minimum years of management and care coordination experience in inpatient settings. The position offers comprehensive benefits and opportunities to shape strategic care delivery.

Qualifications

• Minimum three years' experience in case management/care coordination.
• Minimum three years of recent management experience.
• Previous nursing or social work inpatient experience.

Responsibilities

• Develops and oversees programs to support in-patient care coordination and guides transitional services post hospital discharge with community-based resources to support the social and environmental needs of the patients and their families.
• Provides strong leadership to foster and promote a comprehensive culture of excellence regarding patient experience and quality system wide.
• Identifies opportunities for improvement based on best practice and proactively reaches out to appropriate leadership to communicate findings and initiate needed changes.

Bachelor's degree in nursing or social work
