Job summary

Woodland Springs, part of Lifepoint Health, seeks a Director of Assessment to lead screening and initial assessment for admissions in a 96-bed behavioral health hospital serving Texas communities. You will coordinate with physicians, families, and transportation, ensure HIPAA-compliant documentation, and guide care levels from admission through discharge.

This role requires a Master's in social work or counseling, Texas licensure, and strong clinical leadership; CPR/CPI training within 30 days,

Qualifications

• Master’s degree in social work or counseling and an active Texas license.
• Current unencumbered clinical social work, counselor, or RN license per state requirements.
• Previous management experience in a psychiatric health care facility with assessment experience.
• CPR certification and CPI within 30 days.

Responsibilities

• Directs and performs the screening of potential patients for admission and initiates the integrated assessment process.
• Assists administration, physicians, and clinical staff in assessment of pending admits to determine level of care.
• Uses approved admission criteria to recommend level of care and initiates care plans at admission.
• Coords with physicians, families, transportation, and other facilities to organize admission procedures.
• Ensures accurate referral of vital assessment information to attending physicians with complete history.
• Communicate with external reviewers and referral sources and conduct external reviews per HIPAA guidelines.
