Job summary

Lifepoint Health is seeking an experienced Assessment Director within our Outpatient Therapy team in Washington. You will lead screening, admission assessment and level-of-care decisions to support patient care transitions.

You will coordinate with physicians, families, and external reviewers, ensuring HIPAA compliance and accurate documentation while advancing care quality and safety across the program.

Qualifications

• Master’s degree in social work, counseling or related field required; RN licensure may be used per state guidelines.
• CPR certification and CPI within 30 days.
• Experience in management in a psychiatric health care facility; strong clinical assessment skills.

Responsibilities

• Directs and performs screening of potential patients for admission into the program and initiates the integrated assessment process.
• Assists administration, physicians, and clinical staff in assessing pending admits to determine appropriateness of level of care.
• Make recommendations concerning level of care using the least restrictive option.
• Initiate the plan for episode of care at admission across all levels of care.
• Coordinate with physicians, families, and other health facilities to organize admission procedures.
• Ensure that vital patient assessment information is referred accurately to attending physicians and documented for proper care decisions.
