Job summary

Carrollton Springs, part of Lifepoint Health, is seeking an Assessment Coordinator to support admissions for psychiatric care. This PRN role involves triage, scheduling, patient orientation, and coordination with the Care Coordination Department in Carrollton, TX.

You will perform clinical assessments and process admission paperwork, while maintaining exceptional patient service and coordination with clinicians and referral sources. CPR and de-escalation certs are required within 30 days.

Qualifications

• High school diploma or GED required.
• CPR certification within 30 days of employment.
• De-escalation certification within 30 days of employment.
• Strong customer service and triage skills required.

Responsibilities

• Assesses walk-in and phone callers to determine services or referrals and schedules initial psychiatric assessments.
• Answers inquiries about treatment and facility services, presents options, documents calls, and sets appointments.
• Collaborates with business office to ensure efficient admission process and triage to other sources when appropriate.
• Provides clerical support to Care Coordination Manager and team, including data entry into the electronic record system.
• Maintains stock of forms and supplies for the Care Coordination Department.
• Checks in patient belongings, processes consent forms, and coordinates admission paperwork.
• Schedules assessment appointments and orients patients to units and outpatient programs; coordinates discharges and forwards documents.
