Qualifications

• Master's degree in social work, counseling, psychology, nursing, or a related clinical field; active, unrestricted Michigan clinical license (e.g., RN, LP, LPC, LMSW)
• Bachelor's degree (Master's preferred) in healthcare administration, business, public health, or a related field; minimum 7 years of progressive healthcare operations experience, including multi-site or multi-department leadership
• FQHC, DWIHN, CCBHC, and Michigan regulatory requirements familiarity; ACT fidelity model experience preferred
• Proven ability to lead through organizational change and manage cross-functional handoffs among clinical, case management, and care coordination teams
• BLS required

Responsibilities

• Direct day-to-day clinical and site operations across assigned CCH sites, including patient flow, staffing, scheduling, coverage, and productivity standards.
• Oversee clinical workflows and care coordination among primary care, behavioral health, and dental services.
• Protect the fidelity of the ACT program as an integrated team of case managers, RN, and prescriber.
• Ensure compliance with DWIHN, CCBHC, HRSA, HIPAA, and 42 CFR Part 2; prepare for audits and surveys.
• Partner with Medical Director and other leaders on clinical standards and quality measures.
• Coordinate licensure-track supervision requirements and ensure proper supervision structure.
• Coordinate warm hand-offs and shared-member care with Case Management & Social Health Director.
• Maintain safe, staffed sites; manage budget, supplies, and equipment with Finance.
• Supervise site-based operational and clinical support staff; promote a psychologically safe culture.
• Participate in monthly multi-disciplinary rounds and clinical leadership meetings.
• All other duties as assigned by leadership.
