Job summary

CentraCare seeks a Director of the St. Cloud Hospital at Home (HaH) program to build and scale an integrated, hospital-level care delivery model in patients' homes.

You will oversee nursing, clinical operations, logistics, virtual care, and vendor partnerships, ensuring compliant, high-quality care across settings. You will lead intake, field operations, virtual care, and remote patient monitoring, while managing workforce deployment, budgets, contracts, and regulatory readiness.

Qualifications

• Bachelor's Degree in Nursing required.
• RN license in Minnesota required.
• 3+ years clinical nursing experience in hospital and/or home health care.
• 2+ years progressive leadership experience (supervisor or higher).
• Strong leadership, communication, strategic development, organization and goal setting skills.

Responsibilities

• Lead nursing, operational and administrative staff for HaH program.
• Oversee intake, field operations, virtual care and remote patient monitoring services.
• Manage workforce deployment, budget oversight, contracts and regulatory readiness.
• Liaise between HaH teams and physicians/clinicians to ensure effective communication and coordination.
